The top layer gets pushed ahead of the others by the presser foot while the bottom layer is helped along by the feed dogs. A walking foot is worth the investment. I quilted for over 30 years without a walking foot and it can be done successfully, however my work improved a lot when I got the Pfaff with the dual feed (like a walking foot).
